The Claims Administrator is now accepting Proof of Claim and Release forms (“Proof of Claim“). The deadline to submit a timely Proof of Claim is June 9, 2016.

Your submitted Proof of Claim will also include a release of your legal claims against the Released Defendants (as defined in the Notice), as well as a certification under penalty of perjury that your Proof of Claim is accurate and complete. Please note that supporting documentation of the transactions and holdings listed on your Proof of Claim is required. Include all required and necessary documentation with your submission.
If you fail to execute and submit a timely, properly completed and addressed Proof of Claim, your claim may be rejected and you may be precluded from any recovery from the Net Settlement Fund.
To submit a Proof of Claim by mail, complete and sign the Proof of Claim form in its entirety, with all required supporting documentation and timely mail it to the Claims Administrator at:

No electronic files will be considered to have been properly submitted unless the Claims Administrator issues an email after processing your file with your claim numbers and respective account information. Do not assume that your file has been received or processed until you receive this email. If you do not receive such an email within 10 days of your submission, you should contact the electronic filing department at firstname.lastname@example.org to inquire about your file and confirm it was received and acceptable.
